  • Letter from the relevant country ( or certified copy of foreign Passport)
  • Birth Certificate
  • Original Sri Lankan Passport ( & photocopy)
  • Original Identity Card ( & photocopy)
  • Original Dual Citizenship Certificate ( If available)
  • Original 5(2) Citizenship Certificate ( If available)
