- Form “K”
- Letter from the relevant country ( or certified copy of foreign Passport)
- Birth Certificate
- Original Sri Lankan Passport ( & photocopy)
- Original Identity Card ( & photocopy)
- Original Dual Citizenship Certificate ( If available)
- Original 5(2) Citizenship Certificate ( If available)
All documents should be submitted through the Embassy or Citizenship Division of department of Immigration & Emigration by the applicant.
